ILO is designed by the participant for the participant.

Individualised Living Options (ILO) is a package of support that lets the participants choose where and how they want to live and in a way that best suits them. This really is an exciting prospect, because finally, we have a supported accommodation service that puts the participant at the forefront.

Stage 1
Exploration and Design

Designing the right ILO arrangement for your participant.

After extensive discussions with your participant and their support network about their goals and needs, we can then assist you to complete the steps below:

  • Develop the home and living supports request form

  • Recruit prospective hosts (host can be provided by Carewise)

  • Screening hosts

  • Undertake a home assessments of the host

  • Facilitate meetings between the participant and the host

  • Complete the ILO service proposal form

 


Finding the Right Host
Carewise has a recruitment process in place and manages a pool of competent hosts. If we do not have an appropriate host for your participant we can run a customised recruitment strategy to find a great host for your participant based on their wishes and support needs.

Stage 2
ILO Supports

To deliver stage two, providers must have registration category 0115: Individualised living options - Supports.

Carewise prides itself on delivering high quality supports. We have a robust ILO service model in place that safeguards the participants' living arrangement. With the appropriate policies and procedures in place we can ensure a successful ILO experience for your participant.

  • Establishing an agreement between all parties involved (participants, host, provider)

  • Participant transition to placement

  • Monitoring and safeguarding of the host

  • Training of the host

  • Maintaining regular contact with the participant, host and important others

  • On call service for the host and participant

  • Making payments to the host

A glimpse at an ILO case study.

Here is a case study of an ILO arrangement between Matthew (participant) and Danny (host).

The Participant

Matthew is 32 years old and is currently living at home with his Mother. Matthew is challenge with intellectual disability, mental ill health and psychosocial issues. He is employed by the local supermarket part-time, and loves his footy. Matthew requires support to carry out certain daily living tasks like; assistance with medication, prompting to keep his room tidy, guidance with grocery shopping and travel assistance to get to appointments. Matthew's mother is ageing and can no longer support him in her home. Matthew's main NDIS plan goal is to begin living independently. He has engaged an ILO provider to help him design a living arrangement where he can feel independent, secure and also make new connections whilst still living close to his mother and the community he grew up in.

The Host

Danny is 35 years old, he is currently studying to become a nurse and is an advocate for the empowerment of all groups of people. He has a strong network of connections that share the same values. He is interested in footy, camping and hiking. Danny is renting a home by himself and is interested in becoming a Host. He would like to be part of the support team for a NDIS participant and provide that person with mentorship and guidance. Danny is also interested in earning some extra income and a rental reduction to help him whilst he completes his studies.

The ILO Arrangement

After extensive discussion with Matthew (participant) and the important people in his life, his Support Coordinator and ILO provider, arrange a meet and greet with Danny (Host) to see if they are compatible. The meeting is a success with the two connecting through their shared love of footy and as a result Danny decides that he would like to become a Host for Matthew. The process of making sure Danny is well trained and equipped to become a competent host begins.

Danny agrees to open up his home to Matthew as well as providing approximately 15 hours a week of ILO direct support for things like prompting medication, assistance with domestic duties, shopping for food and cooking meals. Danny is also on hand to provide assistance to navigate and problem solve any challenges and conflicts that might arise for Matthew. As part of Matthews' informal support, his uncle has agreed to call him, drop in to the house from time to time and taking him out for family get-togethers etc.

The ILO provider is coordinating Matthews CP support and stays in constant contact with him to make sure that his health and wellbeing needs are being met. The provider also checks in with Danny regularly to make sure that the home remains a suitable and safe place to live. They will also make sure that Danny remains a well trained, competent and supported host. The ILO provider stays connected with the Support coordinator and Matthew’s support network to keep everyone updated on Matthew's progress.
